Pen/monitor mapping not offered in Wacom user interface

I've had this tablet for years. I have been able to map the pen every time I install the software. Not to say I haven't run into this issue before. I just don't remember what was necessary to fix it. I have the Intuos 3 PTZ-631W tablet. This is the interface I have. Any help would be most appreciated.
