No icon for new Grid Panels or Radial Menus

When I make a new Grid Panel or Radial menu (either through New or Duplicate), it doesn't have an icon associated with it. So when I add it to another grid panel, it can't show an icon, and just shows the name of the file.My hardware is an Intuos Pro small, all drivers and software are up to date.
